Gerrie Gardner, DO, FACC, FACP, is a practicing invasive cardiologist with Providence Medical Group (PMG) in Everett, WA. Her focus is heart failure, women’s heart health, and device technology. She is the chair of the Heart Failure Focus Group for the Providence Region in WA, CA, OR, MT and AK. The aim of the group is to decrease heart failure readmissions and mortality. One of her other goals is to improve the overall cardiac health of patients in the Northwest US. She recently received the PMG Leadership award for her work on cardiology initiatives.

Mona M. Counts, PhD, NP-C, CRNP, FNAP, FAANP, FAAN

Mona M. Counts, PhD, NP-C, CRNP, FNAP, FAANP, FAAN, is Elouise Ross Eberly professor emeritus, Pennsylvania State University College of Nursing in University Park, PA. She is one of the original nurse practitioners in the country, and helped develop the NP program at Emory University in 1970. Since that time, Dr. Counts has developed programs at a number of universities across the nation, including Penn State.

Donald L. Deye, MD, FACP

Dr. Deye is a general internist who has practiced for over 30 years at Cambridge Medical Center (CMC) in Cambridge, MN. He is a Fellow of the American College of Physicians (ACP), Director of Continuing Medical Education for CMC and the Chair of the Pharmacy and Therapeutics Committee for both CMC and Allina Medical Clinics. Currently, Dr. Deye serves as Isanti County (MN) Public Health Physician, on the Editorial Advisory Board of the Minnesota Medical Association’s (MMA) acclaimed journal, Minnesota Medicine, on the Preferred One Physician Associates (MN) Board of Directors and Nominating Committee, and as Clinical Preceptor for PA, NP and medical students. Dr. Deye is a member of the Clinical Practice Council for Allina Health in Minnesota, the Medical Policy Committee for Medica (of Minnesota). He was a poster competition judge at both the Minnesota and national meetings of the ACP and a Research Consultant for Resolution Health. Serving as Chief Medical Officer for Oakstone Publishing, Dr. Deye is also Creator, Editor, and Host of Oakstone’s award-winning MKSAP Audio Companion, sponsored by the ACP. He is Creator and Founding Editor of Oakstone’s audio journal club, Practical Reviews in Hospital Medicine, where he continues to be heard monthly in the “Something Else” segment at the end of each issue. Dr. Deye is the immediate past District Medical Director for the North Region of Allina Medical Clinic, past President of the East Central Minnesota Medical Society and of the Minnesota Coroners and Medical Examiners Association, and past County Coroner for Isanti County. He served on the Governor’s Council for the Minnesota Chapter of the ACP and as Delegate to the MMA’s Annual Convention representing the Minnesota Chapter of the ACP. He has also served on the Hormone Replacement and Menopause Working Group for the Institute for Clinical Systems Improvement (ICSI) and served on the Adjunct Associate Faculty for the University of Minnesota’s RPAP (Rural Physician Associate Program) teaching medical students. Dr. Deye chaired and served on reference committees for the MMA and has taught NP and PA students from several institutions in the upper Midwest. Dr. Deye attended Valparaiso University where he was in the Charter Class of the Christ College Honors Program, was in the Charter Class of Mayo Medical School and completed residency in Internal Medicine at the Mayo Graduate School of Medicine.

Benjamin Frizner, MD, MBA, FHM

Benjamin Frizner, MD, MBA, FHM, has over 10 years' experience working in the hospital, outpatient, and skilled nursing settings. He was the director of hospital medicine at Saint Agnes Hospital in Baltimore for four years before becoming the director of the long-term ventilator unit at FutureCare Irvington. He also serves as the director of quality and performance for CEP America and recently became the medical director for FutureCare Homewood and Irvington. At FutureCare Irvington, Dr. Frizner has decreased unplanned discharges and readmissions by collaborating with local emergency departments. When Dr. Frizner was a hospitalist director, he worked to improve DVT prophylaxis, decrease urine catheter usage, develop post-fall evaluation protocols, and improve surgical co-management. While working in the post-acute care setting, Dr. Frizner collaborated with nursing to develop an antibiotic stewardship program, daily rounds, and a blood transfusion protocol. Since Dr. Frizner has been working at Irvington, patient and family satisfaction has improved through scheduled care plan meetings with social work and the rehab department.

Peter Gross, MD

Peter Gross, MD, is the chair of the HackensackAlliance Accountable Care Organization's Board of Managers. Recognized as a high performing ACO, Hackensack Alliance has earned nearly $24 million in Medicare shared savings over four years. Dr. Gross is also professor of medicine, preventive medicine and community health at UMDNJ-New Jersey Medical School. Dr. Gross's past leadership positions include: chair of the Sentinel Event Alert Advisory Committee at The Joint Commission; chairman of the practice guidelines committee of the Infectious Disease Society of America; president of the Society of Healthcare Epidemiologists of America; and chair, Drug Safety and Risk Management Advisory Committee at the FDA.

Susan Kay Lee, DNP, NP-C, FAANP

Susan Kay Lee, DNP, NP-C, FAANP has been a nurse practitioner for more than 20 years. She received her doctorate in nursing practice from George Washington University in 2012. Ms. Lee is part-time clinical faculty at the Indiana University School of Nursing. She serves on the board of directors of the Coalition of Advanced Practice Nurses of Indiana and is certified by the American Association of Nurse Practitioners. She has a special interest in preventive care.

Monique Lucarelli, MD

Monique Lucarelli, MD, has spent more than 20 years practicing both primary care and hospitalist medicine. In 2017 her work expanded into the realm of post-acute care. Board certified in internal medicine, Dr. Lucarelli lives in North Carolina with her husband and dog Pearl.

Leslie Milteer, PA-C, MPAS, DFAAPA, is a Physician Assistant (PA) with over 19 years of medical practice experience, the majority spent in primary care with a focus on preventative care and chronic disease management. In addition to clinical practice PA Milteer has been an assistant professor at Saint Catherine University Master of PA Studies Program since 2014, served on the Institute for Clinical System Improvement Adult and Pediatric Preventative Care Workgroup since 2007, and was a member of the 2017 NCQA Patient Centered Medical Home Advisory Committee. PA Milteer has been awarded Distinguished Fellow Status from the American Academy of PAs in recognition of her years of quality medical practice, leadership roles, and professional accomplishments.

Larissa Lucas MD, FACP, HMDC, is medical director of quality at Care Dimensions, and instructor of medicine at Harvard Medical School in Boston. At Care Dimensions, Dr. Lucas focuses on patient safety, evidence-based practice, and team approaches to care at end of life. Previously, she managed a large team of editors building the evidence-based educational resource Dynamed. Dr. Lucas is board certified in internal medicine and as a hospice medical director, and is a fellow of the American College of Physicians.
